SHADOW DAY RELEASE

I am the parent and/or legally authorized guardian of the child participating in the Mater Dei High School (“MDHS”) Shadow Program on. On behalf of myself, my spouse, the Student, and my/our heirs, personal representatives, and assigns, and in consideration for the Student to be allowed to take part in the Shadow Program, I make the following representations.

I will direct the Student to follow all instructions and guidelines provided by MDHS faculty and staff during the Shadow Day. I hereby release and hold harmless MDHS and the Catholic Diocese of Orange, as well as their respective employees, volunteers, directors, affiliates, and representatives, from any and all liability, damages, and causes of action of every kind whatsoever relating in any way to the Student’s participation in the Shadow Program. I am unaware of any health issue or restriction that would affect the Student’s participation in the Shadow Program. In the event of an emergency, I authorize MDHS to act for me in its best judgment in rendering any medical attention to the Student.

I understand that MDHS may take photographs, film footage, and other electronic media as part of the Shadow Program, some of which may contain the image, likeness or voice of the Student. I grant permission to MDHS to use this media for its educational and advertising purposes and understand that neither the Student nor his/her family will be financially remunerated for this media usage.
